SEB Administrative Services Inc. has been a Third Party Administrator (TPA) in the marketplace, since spring, 2006. We provide group benefits to our clients, & process & adjudicate Extended Health Care (EHC) claims, Dental claims & Pharmacy (Drug) claims, both electronically & manually.
No. The Dental Provider Agreement is the Agreement between the Provider & SEB Administrative Services Inc., that ensure that payment will be made to the Provider (you) for services rendered to SEB Administrative Services Inc. clients, when clients present their SEB Administrative Services Inc. cards to you.
Yes. The Dental Provider Registration Form must be completed so that SEB Administrative Services Inc. can register you to have the ability to submit claims electronically. Claims made available electronically can be made to the insured or the service provider.
Yes. Your Dental Provider number is the same as your CDAnet registration number.
Your office ID number is the office ID number assigned by CDAnet. If you do not know your ID number, please contact our Customer Service Centre at 416-637-3484 or 1-888-939-8885, Monday to Friday, 8:00am – 5:00pm EST or you can login to the secure member portal.
The Software Version Number is the claim that you support in claim submission i.e.: 2 or 4.
Your Software Device is the name of the application that is used in the service provider office (Dental office or Pharmacy).
A RX/Carrier ID number & a BIN number is required to transmit all electronic claims.
